I guess this post is a great case study of usability and UX. A lot of do's and dont's for pfecert forms. Some of these draw my attention, specially #14 which the cancel is a button-like too close of the positive action (sign in). It's not wrong because the grey color and the left position are trying to avoid user's mistakes. But it could be better turning it into a link. And I agree with some opinions above.
